Q:
How do I refer a patient to your office? Most patients
contact the office directly by calling 386-290-2993. If you
are referring a patient please suggest they contact the office directly to
schedule an appointment. If you are a
social worker from an inpatient facility providing discharge planning, you may
contact this office on behalf of the patient to secure an appointment
post-discharge from the inpatient facility. This office
also accepts referrals from probation, parole, and child protective service
agencies. If requested by these referral
sources, this office will make the first contact with the patient. Q:
How long will it take from referral call to your office to the patient's
first session with you? Most patients
are seen for their first session within seven business days of the first phone call
for services. This can vary when Kyle Gilrain, or the patient, is on vacation or not available for
some days of a given week. Q: If I refer a patient to your office
will I ever hear from you about that patient? In some cases
a patient is referred and the referral source does not need any follow up or
updates on the patient’s status. In
these cases, you will not hear from this office about a patient that has been
referred. It must be both necessary and
approved by the patient for this office to disclose any information about a
patient that has been referred to this office for treatment. In some
cases, and with the patients signed written consent, this office has several
methods for follow up with referral sources.
By way of illustration, but not exhaustion, for those patients court ordered
to attend and participate in treatment at this office, the referral source will
be provided with a basic summary on a regular basis. Further, the summary can be requested at any
time provided there is a written consent on file with this office.
In addition, for patients who provide consent and are required to regularly attend
treatment by an ancillary agency, missed treatment letters will be sent
directly following (same day) for any missed appointments. This office does not determine the validity
of the reason given, if any, for missed appointments. The reason provided is, however, provided in
the missed appointment letter.
